The Department of Revenue provides taxpayers with the ability to request common Pennsylvania tax forms. The form(s) will be mailed directly to the taxpayer address that is provided with the order request. If the requested form(s) is not listed on the ordering site, you are encouraged to review the Forms and Publications section on the Revenue website by visiting  revenue.pa.gov/forms. If you have any issues ordering or finding a specific form, you can email ra-forms@pa.gov.

Did you file a paper Property Tax/Rent Rebate application and receive a rebate within the last year?

You do not need to request a paper application. A paper application will be mailed to the address listed on last year's application.


Did you use myPATH to file a Property Tax/Rent Rebate application and receive a rebate within the last year?

We strongly encourage you to again use myPATH to file your rebate application online. The department will mail you a copy of the rent certificate to use with your online application. You do not need to request a paper application.


The Department of Revenue strongly encourages applicants to file their rebate applications online using myPATH (mypath.pa.gov). This is the department’s user-friendly online filing system. Filing online gives you instant confirmation that a claim has been successfully filed, allows access to automatic calculators, and includes other helpful features.
